Job Description:

PROGRAM OVERVIEW
The North County Senior Homeless Program (NCSHP) is a new Full Service Partnership (FSP) serving 30 older adults with a severe and persistent mental illness who are homeless or at risk of homelessness in Northern Alameda County. The goal of the program is to reduce homelessness and maximize independence for these older adults. NCSHP is based on the Assertive Community Treatment (ACT) model and provides intensive services in the context of a multi-disciplinary team. Team members work together to do “whatever it takes” to provide services which assist consumers to reach their goals of wellness and recovery, including finding and maintaining stable housing. Services will be predominantly field-based rather than office-based, and service coverage is 24/7."
